Complete MMM Exp Form 1 and CM/ECF Limited Filer Account Registration Form, if required, and file with the Clerk's Office at 230 N 1st Ave, Ste 101, Phoenix, AZ 85003 or 38 S Scott Ave, Ste 100, Tucson, AZ 85701. Your application will be reviewed and you will be notified by the Clerk's Office pending approval.
Pursuant to Section IV(C) of the MMM Procedures, the official MMM Forms provided below shall be used without material alteration. The MMM Forms are provided as pdf fillable forms.
